What does this stuff do? I'm assuming it's ot another firewall or antivirus but something else?

They check for spyware and adware, they range from annoying such as spawning pop-ups to a major security risk such as recording passwords and keystrokes and sending them across the web.
Personally I'd run both Spybot S&D and Ad-Aware to make sure you get everything.
